What are your office hours?
A. Our office hours are 9:00 am to 5:00 pm, Monday through Friday. However, we are closed on the following holidays: January 1st, Good Friday, July 4th, Labor Day, Thanksgiving and the following Friday, and Christmas Day
How do I reschedule or cancel an appointment?
A. Usually you must give us 48 hours notice so that we may allow other patients the opportunity to take your appointment and, if necessary, reschedule your visit. However, it is most courteous to call as soon as you realize you must reschedule or cancel. Please try to be on time for your appointment. If you are running late, call ahead and let the hospital/clinic know your estimated time of arrival.
What are the billing and insurance policies?
A. Referrals, Pre-Authorization & Second Opinions Most insurance companies require pre-authorization on many procedures. It is important for patients to know the requirements of their insurance. Second opinions also may be needed before procedures are performed. If your plan requires a referral from your primary care physician it is your responsibility to obtain this prior to each visit. We accept the following insurances.

How do I get my medical records released?
A. We take your privacy seriously and require that all requests for a release of medical records are accompanied with a filled out and signed Medical Release Form. Please call our office to obtain a form or to answer any questions about how to release your medical record.
